Home Health PTA - Part Time

About the role

As a Home Health Physical Therapist Assistant, you will deliver personalized rehabilitation care to patients in their homes, focusing on improving strength, mobility, and independence. You will execute treatment plans created by Physical Therapists, utilizing manual exercises, therapeutic modalities, and other techniques. Your responsibilities include documenting patient progress, reporting observations, and collaborating with healthcare team members for care coordination and discharge planning.

Qualifications

• Valid, unrestricted Physical Therapy Assistant license in the designated state
• Valid driver’s license, auto insurance, and reliable transportation
• Current CPR certification
• Preferred: Home health experience

Responsibilities

• Administer physical therapy treatments as per plan
• Document patient progress and clinical notes
• Support care coordination and discharge activities

Additional Requirements

• Must adhere to driver safety policies
• Participate in TB screening
• Maintain compliance with health and safety protocols
