High Needs Care Coordinator

Casa de los Niños is one of the most established and well respected non-profit organizations in Tucson, helping kids and healing families since 1973. Services include parent education classes, Foster Care, Parents As Teachers, Behavioral Health, and Nurse-Family Partnership Program.

Responsibilities

  • Coordinate behavioral health services for children with complex psychosocial needs, providing individual, family, and group services to children, youth, and families.
  • Facilitate Child & Family Teams to engage members and their families in recovery-oriented treatment planning.
  • Manage an assigned caseload, delivering services individually and as part of a team.
  • Maintain accurate and timely clinical service records, prepare and provide reports, and submit data promptly.
  • Demonstrate ability to maintain confidential/sensitive information.
  • Engage members and their families or legal guardians in service planning to meet the needs of children enrolled in the behavioral health system.
  • Provide individual, family, and group services under the direction of a qualified Behavioral Health Professional or Technician.
  • Provide intake and completion of a Core Assessment.
  • Identify needs with families and complete service plan with member and family/guardian.
  • Facilitate Child & Family Teams to review service needs, treatment planning progress, goal attainment, and utilization of services and natural supports.
  • Coordinate a comprehensive array of services and accompanying referral and administrative requirements for service delivery.
  • Provide services related to the acuity of the client's needs.
  • Provide positive, proactive communication to other system partners related to care coordination needs.
  • Provide complete documentation that accurately reflects services delivered in accordance with regulatory, contractual, and agency requirements.
  • Assist with Case Management functions with a caseload of 20 and meet program productivity standards.
  • Provide effective crisis management and maintain necessary documentation of service delivery.
  • Conduct educational activities that promote client understanding of treatment issues.
  • Interface with DCS, the courts and other partners regarding child well-being, family stability and case planning.
  • Plan for client discharge/termination, including helping the family plan.
  • Maintain clinical records in a timely manner following agency standards.

Requirements

  • At least 21 years of age (licensing requirement).
  • Bachelor's degree in social work, psychology, or related subject area.
  • High school diploma may be considered with relevant years of behavioral health experience that comply with Office of Behavioral Health Services staff qualification requirements.
  • CPR/First Aid certification required.
  • Must be able to provide own transportation.
  • Must be available to work after hours, weekends, and on-call on a rotating basis.
  • Must be able to meet training and agency requirements for the position.

  • Must possess or be able to qualify for and maintain a valid Arizona Department of Public Safety Level One fingerprint clearance card.
  • Must possess and maintain a valid Arizona driver's license and be insurable under the agency's automobile policy.
  • Bilingual in Spanish preferred, but not required.
