About the role
Care Coordinator for the Birth to Three Program, supporting children aged 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Partner with families, service providers, and community resources to promote child development.
- Conduct functional assessments and develop individualized, family-centered service plans.
- Coordinate and facilitate services aligned with care plans.
- Support families through education, advocacy, and collaboration.
- Maintain accurate documentation and facilitate family-centered team meetings.
- Collaborate with internal teams and community providers.
- Manage a flexible schedule, including home visits and occasional evening or weekend hours.
- Meet productivity and service delivery expectations.
Qualifications
- Bachelor's degree in Social Work, Child Development, or related human services field.
- Experience working with families with special needs preferred.
- Valid driver's license, reliable transportation, and ability to work in diverse settings.
- Strong organizational, communication, and documentation skills.
- Ability to respond effectively in crisis situations.
Work Environment & Skills
