About the role

Care Coordinator for the Birth to Three Program, supporting children aged 0–3 with developmental delays or disabilities and their families.

Responsibilities

  • Partner with families, service providers, and community resources to promote child development.
  • Conduct functional assessments and develop individualized, family-centered service plans.
  • Coordinate and facilitate services aligned with care plans.
  • Support families through education, advocacy, and collaboration.
  • Maintain accurate documentation and facilitate family-centered team meetings.
  • Collaborate with internal teams and community providers.
  • Manage a flexible schedule, including home visits and occasional evening or weekend hours.
  • Meet productivity and service delivery expectations.

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related human services field.
  • Experience working with families with special needs preferred.
  • Valid driver's license, reliable transportation, and ability to work in diverse settings.
  • Strong organizational, communication, and documentation skills.
  • Ability to respond effectively in crisis situations.

Work Environment & Skills
