About the role
Care Coordinator for the Birth to Three Program, a federally mandated Early Intervention initiative supporting children ages 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Partner with families, service providers, and community resources to support child development.
- Conduct functional assessments and develop individualized, family-centered service plans.
- Coordinate and facilitate services in line with care plans.
- Provide family education, advocacy, and collaboration.
- Maintain accurate documentation compliant with regulations.
- Lead family-centered team meetings and collaborate with internal teams and community providers.
- Manage a flexible schedule based on client needs, including occasional evenings or weekends.
- Meet productivity and service delivery expectations.
Qualifications
- Bachelor's degree in Social Work, Child Development, or related human services field.
- Preferred: at least one year of supervised experience working with families with special needs.
- Valid driver's license, reliable transportation, and ability to meet driving and insurance requirements.
- Strong documentation, communication, and organizational skills.
- Comfort working in home, office, and community settings.
- Ability to respond appropriately in crisis situations.
Work Environment & Skills
Community-based role involving daily travel, moderate physical activity, and exposure to various environments. Ability to work in diverse settings and respond to crises.
