Patient Care Coordinator

About the role

We are seeking a Patient Care Administrator (PCA) to join the senior leadership team, overseeing the clinical, financial, and operational aspects of hospice services within a designated area. In this role, you will support interdisciplinary team members including nurses, social workers, and chaplains, coach staff, consult on clinical matters, and lead performance improvement initiatives to enhance patient experience. The ideal candidate will blend leadership skills and clinical expertise to ensure quality and continuity of care.

Responsibilities

• Oversee clinical, financial, and operational aspects of hospice services
• Support interdisciplinary team members (nurses, social workers, chaplains)
• Coach staff and consult on clinical matters
• Lead performance improvement initiatives to enhance patient experience

Requirements

• Active RN license
• At least four years of healthcare management experience
• Hands-on fiscal and budgetary experience
• Previous experience managing teams
• Commitment to compassionate patient care

Qualifications

• Bachelor’s degree in Nursing or related field (preferred)
• Relevant clinical and supervisory experience (varies by state)
