Responsibilities

  • Provide care coordination to agency patients or persons requesting services, working one-on-one to help them better self-manage their health through coordination with providers and education on local resources.
  • Conduct outreach and initial Community Needs Assessments, developing a client profile and initial checklist. Administer health screens such as the Patient Activation or Caregiver Activation Measure (PAM/CAM), Patient Health Questionnaire (PHQ-9), and the KATZ measure for activities of daily living. Update the patient checklist at every visit.
  • Support patients in attaining short and long-term goals using motivational interviewing techniques. Advocate, educate, and support patients and their natural family supports to improve self-management skills.
  • Act as a cultural mediator between communities and the healthcare service system. Provide culturally appropriate and accessible health education, informal counseling, social support, and advocacy for individual and community needs.
  • Build trust with community members, local service organizations, and healthcare providers to enhance individual and community capacity.
  • Meet with patients both in person and telephonically to support healthcare goals. Complete a minimum of one home visit or community visit with participants every 30 days, adhering to agency physical distancing guidelines.
  • Coordinate with authorizing healthcare or social service entities to maintain self-care and support patient goals.
  • Document patient progress, recommendations, and all in-person and telephonic encounters.
  • Attend HealthConnect monthly meetings and required trainings. Utilize HealthConnect/HUB Connect Software to track referrals.
  • Carry a caseload (e.g., 40-60 moderate-risk community members or 25 very high-risk individuals). Caseload size may vary based on duties and acuity in consultation with the Program Director.
  • Perform other duties as assigned.

Requirements

  • High school diploma or equivalent and 2 years of experience working in a related field/profession.
  • Associates or Bachelor's degree in a related social service field preferred but not required.
  • Above-average Microsoft Office skill set.
  • Experience working within health systems or providing case management.
  • Experience with underserved, transient populations.
  • Unencumbered driver’s license and ability to become an approved driver per agency policy.
  • Familiarity with Motivational Interviewing techniques.
  • Strong communication skills.
  • Valid driver’s license with a good driving record and an insured private vehicle to fulfill job duties.

Work Environment

  • Work is primarily performed while sitting and talking, with travel required within the community.
  • Working hours are spent indoors in the community, patient homes, and at community agencies. Participation in staff retreats and development activities is required.
  • Dress appropriately for home or community settings (e.g., avoiding high heels or expensive jewelry).
  • Carry an agency-issued cell phone during in-person visits for safety, medical issues, and scheduling support.

Pay

$20.65 - $30.98 per hour, depending on experience.
