Location: Winchester, VA (Northern Virginia region). Applicants must reside in Winchester or Shenandoah, VA. Travel may include up to 90 minutes one way.

About the role

Sentara Health Plans is hiring a full-time (40 hours/week) Care Coordinator who will perform both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+) and Substance Abuse Disorders.

Standard working hours: 8:00 AM to 5:00 PM EST, Monday through Friday.

Responsibilities

  • Perform a variety of casework duties and provide case management services to patients, families, and designated caregivers.
  • Develop, participate in, and monitor multidisciplinary collaboration of services to patients where appropriate.
  • Assist adult patients and their families with personal and environmental difficulties associated with medical conditions, up to and including terminal illnesses.

Requirements

Education

Must possess a degree in Health & Human Services or one of the following related fields from an accredited institution (as listed on the U.S. Department of Education College Accreditation database):

  • Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)
  • Counselor Education
  • Early Childhood Development
  • Educational Psychology

  • Marriage and Family Therapy
  • Music Therapy
  • Nursing
  • Pharmacy
  • Psychiatric Rehabilitation

  • Therapeutic Recreation

Degrees obtained from schools outside the United States will be reviewed individually.

Experience

  • 1 year of Long Term Care experience (required)
  • 1 year of Health Plan experience (preferred)
  • 1 year of Medicaid experience (preferred)
  • At least 1 year of mental health experience (preferred)
  • 1 year of experience with Children/Adolescents (required)
