About the role
Care Coordinator for the Birth to Three Program, a federally mandated Early Intervention service supporting children ages 0–3 with developmental delays or disabilities and their families.
Responsibilities
- Conduct functional assessments and develop individualized, family-centered service plans
- Coordinate and facilitate services
- Support families through education and advocacy
- Maintain accurate documentation
- Lead family-centered team meetings
- Collaborate with internal and community providers
- Manage a flexible schedule
- Meet productivity expectations
Qualifications
- Bachelor's degree in Social Work, Child Development, or related field
- Experience working with families with special needs preferred
- Valid driver’s license and reliable transportation
- Strong communication, organizational, and documentation skills
- Ability to work in diverse settings and respond effectively in crises
Work Environment & Skills
- Community-based role involving travel and physical activity
- Comfortable working in homes, offices, and community settings
- Ability to adapt to various environments and occasional schedule flexibility
