About the role

Care Coordinator for the Birth to Three Program, a federally mandated Early Intervention service supporting children ages 0–3 with developmental delays or disabilities and their families.

Responsibilities

  • Conduct functional assessments and develop individualized, family-centered service plans
  • Coordinate and facilitate services
  • Support families through education and advocacy
  • Maintain accurate documentation
  • Lead family-centered team meetings
  • Collaborate with internal and community providers
  • Manage a flexible schedule
  • Meet productivity expectations

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related field
  • Experience working with families with special needs preferred
  • Valid driver’s license and reliable transportation
  • Strong communication, organizational, and documentation skills
  • Ability to work in diverse settings and respond effectively in crises

Work Environment & Skills

  • Community-based role involving travel and physical activity
  • Comfortable working in homes, offices, and community settings
  • Ability to adapt to various environments and occasional schedule flexibility
