A full-time Physician position in a collaborative primary care practice.

Responsibilities

• Provide comprehensive primary care to patients of all ages, including diagnosis and management of acute and chronic conditions.
• Perform preventive care, wellness visits, and age-appropriate screenings.
• Ensure continuity of care across life stages, supporting individual and family health.
• Collaborate with healthcare team members to deliver coordinated, team-based care.
• Utilize electronic health records, telehealth, and patient portals to improve access and efficiency.
• Participate in quality improvement and practice development initiatives.
• Educate patients and families on health promotion and disease prevention.

Requirements

• Medical degree (MD or DO) with completion of internship and residency.
• Active medical license in NYS.
• Board certification or eligibility in an ABMS or AOA specialty.
• Interest in building long-term patient relationships and working in a team environment.
