About the Role
Full-time Physician in a collaborative primary care practice, providing comprehensive care across all ages. Deliver primary care services including diagnosis, management of acute and chronic conditions, preventive care, wellness visits, and screenings. Support continuity of care for patients and families, promoting long-term relationships and high-quality outcomes.
Collaborate with a team of physicians, nurse practitioners, and physician assistants to ensure coordinated, patient-centered care. Utilize electronic health records, telehealth, and patient portals to improve access and efficiency. Participate in quality improvement initiatives and practice development. Educate patients on health promotion and disease prevention.
Responsibilities
- Providing comprehensive primary care across the lifespan
- Diagnosing and managing acute and chronic conditions
- Delivering preventive care, wellness visits, and screenings
- Fostering long-term patient relationships and continuity of care
- Collaborating with a multidisciplinary healthcare team
- Utilizing electronic health records, telehealth, and patient portals
- Participating in quality improvement and practice development initiatives
- Educating patients on health promotion and disease prevention
Qualifications
- MD or DO degree
- Accredited internship/residency
- Active New York State medical license
- Board certification by an ABMS Member Board or AOA specialty board (recent graduates with board eligibility considered)
