Epilepsy Nurse Navigator (RN)

Role: The Nurse Navigator (NN) is a licensed registered nurse with disease-specific clinical expertise, providing individualized support to patients, families, and caregivers to overcome care barriers. Responsibilities: Serve as a subject matter expert, improve care access and timeliness, collaborate with program leaders, and deliver patient-centered care using the nursing process. Coordinate multidisciplinary care, identify service gaps, and promote evidence-based practice. Support performance improvement initiatives, mentor staff, and ensure compliance with organizational policies and standards.Qualifications & Skills: Bachelor's or higher in Nursing, valid RN license, and certifications in ACLS and BLS are required. Disease-specific certification within one year is necessary. Minimum five years of nursing experience, with at least two in disease-specific or outpatient care, along with strong critical thinking, time management, and emergency response skills. Ability to work in a fast-paced environment and foster collaboration are essential. Key Duties: Facilitate access to care, coordinate high-level care activities, participate in quality improvement, and act as a mentor. Maintain ongoing competency, stay current on oncology advances, and ensure adherence to safety and regulatory standards. This role is patient- and provider-facing, utilizing in-person, telephonic, and electronic communication methods.
