Epilepsy Nurse Navigator (RN)

Role: The Nurse Navigator (NN) is a licensed registered nurse specializing in disease-specific care, providing individualized support to patients, families, and caregivers to facilitate access to quality healthcare throughout the cancer continuum. Responsibilities Serve as an expert in patient navigation, improving care timeliness and access.Collaborate with leadership to assess community needs and enhance programs.Deliver patient-centered care using the nursing process, including assessment, diagnosis, planning, implementation, and evaluation.Coordinate multidisciplinary care, ensure documentation, and promote inclusive communication.Support performance improvement initiatives and participate in professional development.Mentor and precept other nurses, stay current with oncology advances, and uphold organizational policies and safety standards. Qualifications & Skills Bachelor’s or Master’s degree in Nursing.RN license with required certifications (BLS, ACLS) upon hire; disease-specific certification within one year.Minimum 5 years of nursing experience, including 2 years in disease-specific care and outpatient or care coordination preferred.Strong critical thinking, time management, and ability to respond decisively in fast-paced environments.
