Epilepsy Nurse Navigator (RN)

Role Overview

The Nurse Navigator (NN) is a licensed registered nurse specializing in disease-specific care, providing individualized support to patients, families, and caregivers to overcome barriers to care.

Responsibilities

• Serve as subject matter expert in navigation, improve access to care, coordinate multidisciplinary care, and promote timely patient access and adherence to treatment plans.
• Utilize the nursing process—assessment, diagnosis, planning, implementation, and evaluation—to deliver evidence-based, patient-centered care.
• Collaborate with teams, participate in quality improvement, and support program growth, ensuring culturally competent and inclusive services.
• Mentor and serve as preceptor for nursing staff and students, staying current with oncology advancements, and adhering to organizational policies and safety standards.

Qualifications & Skills

• Minimum of 5 years of nursing experience, with 2 years in disease-specific care, outpatient or care coordination preferred.
• Must hold active BLS and ACLS certifications, with ability to work in a fast-paced environment, demonstrate critical thinking, and respond effectively to emergencies.
