Epilepsy Nurse Navigator (RN)

About the Role

The Nurse Navigator (NN) is a licensed registered nurse with disease-specific expertise, providing individualized support to patients, families, and caregivers to improve care access and adherence throughout the cancer continuum.

Responsibilities

• Serve as a subject matter expert in disease-specific care.
• Coordinate care and enhance patient access to timely, quality services.
• Utilize the nursing process (assessment, diagnosis, planning, implementation, evaluation) to deliver evidence-based, patient-centered care.
• Collaborate with multidisciplinary teams to support program growth and performance improvement.
• Mentor staff and ensure compliance with organizational policies, standards, and certifications.

Requirements

• Bachelor's or master's degree in nursing.
• Valid RN license.
• Certifications in BLS, ACLS, and disease-specific area (to be achieved within 1 year).
• Minimum of 5 years of nursing experience, including 2 years in disease-specific care, outpatient care, or care coordination.
• Ability to work across various communication platforms and demonstrate teamwork.
• Uphold organizational policies and participate in ongoing professional development and quality initiatives.
