Role: The Nurse Navigator (NN) is a licensed registered nurse with disease-specific expertise, providing personalized support to patients, families, and caregivers to overcome care barriers. Responsibilities: Serve as a subject matter expert in navigation, enhancing access and timeliness of care.Collaborate with program leaders to evaluate community needs and improve services.Deliver patient-centered care tailored to diverse populations, using the nursing process for assessment, diagnosis, planning, implementation, and evaluation.Coordinate care across specialty clinics and multidisciplinary teams, bridging service gaps.Promote safe, quality care through interdisciplinary communication, documentation, and team collaboration.Participate in performance improvement, education, mentoring, and professional development activities.Ensure compliance with organizational policies, safety standards, and accreditation requirements. Qualifications & Skills: Bachelor's or Master's in Nursing preferred; valid RN license required.Certifications in ACLS, BLS, and disease-specific areas (to be obtained within 1 year).Minimum 5 years nursing experience, with 2 years in disease-specific care; outpatient or care coordination experience preferred.Strong critical thinking, time management, and ability to respond decisively in fast-paced settings.Excellent interpersonal and communication skills to work effectively with diverse teams and patient populations.
