Role Overview
Support individuals in maintaining independence and self-direction through the IRIS program by building trusting relationships, developing support plans, and connecting clients with community resources. Work independently within a social services capacity while collaborating with a team.
Responsibilities
- Build rapport with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Assist participants in understanding IRIS eligibility
- Coordinate with local agencies and ADRCs
- Document all interactions promptly
- Conduct annual Long-Term Care Functional Screens (if applicable)
Qualifications & Skills
- Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interaction, and community resources
- Willingness to serve diverse populations with empathy
- Valid driver’s license and reliable transportation
