Location: Milwaukee, WI (Hybrid Role) • Full-Time, 40 hrs/week • Benefits Eligible • Travel Required: 30–50%

Role Overview

Support individuals in maintaining independence and self-direction through the IRIS program by building trusting relationships, developing support plans, and connecting clients with community resources. Work independently within a social services capacity while collaborating with a team.

Responsibilities

  • Build rapport with participants
  • Develop IRIS Support and Services Plans
  • Maintain regular contact and conduct visits
  • Assist participants in understanding IRIS eligibility
  • Coordinate with local agencies and ADRCs
  • Document all interactions promptly
  • Conduct annual Long-Term Care Functional Screens (if applicable)

Qualifications & Skills

  • Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
  • Strong commitment to participant autonomy and respectful service
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior, social interaction, and community resources
  • Willingness to serve diverse populations with empathy
  • Valid driver’s license and reliable transportation
