About the Role
Join us as an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.
Responsibilities
- Building trusting relationships with participants
- Assisting in developing IRIS Support and Services Plans
- Conducting monthly contacts and quarterly visits
- Helping participants understand IRIS eligibility
- Connecting with local agencies to broaden service options
- Documenting interactions within required systems promptly
- Potentially conducting annual functional screens
- Working independently with team collaboration
Qualifications and Skills
- Bachelor’s degree in social work, psychology, or related fields and at least one year of supervised experience with seniors or disabilities; or 4+ years of relevant social services experience; or current IRIS Consultant in good standing
- Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Willingness to serve diverse populations with empathy
Requirements
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
