Disability Services Consultant - Bachelor's Required

About the Role

Join us as an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.

Responsibilities

• Building trusting relationships with participants
• Assisting in developing IRIS Support and Services Plans
• Conducting monthly contacts and quarterly visits
• Helping participants understand IRIS eligibility
• Connecting with local agencies to broaden service options
• Documenting interactions within required systems promptly
• Potentially conducting annual functional screens
• Working independently with team collaboration

Qualifications and Skills

• Bachelor’s degree in social work, psychology, or related fields and at least one year of supervised experience with seniors or disabilities; or 4+ years of relevant social services experience; or current IRIS Consultant in good standing
• Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
• Strong commitment to participant autonomy and respectful service
• Proficiency with smartphones, tablets, and computer systems
• Knowledge of human behavior, social interactions, and community resources
• Willingness to serve diverse populations with empathy

Requirements

• Valid driver’s license and reliable transportation
• Satisfactory driving record and auto insurance compliance
