About the role
We are seeking an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Conduct regular contact and visits to ensure participant needs are met
- Assist with understanding and maintaining IRIS eligibility
- Connect participants with local agencies and resources
- Accurately document all interactions within required systems
- Possibly conduct annual functional screens
- Work independently while collaborating with a team
Qualifications
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience; or current IRIS Consultant in good standing
- Bilingual skills are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of social behavior, community resources, and cultural competence
- Valid driver’s license and reliable transportation
