About the role
Join as an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Assist participants with IRIS eligibility and connect them with local agencies
- Document all interactions in required systems promptly
- Possibly conduct annual Long-Term Care Functional Screens
- Collaborate with a supportive team while working independently
Qualifications
- Bachelor’s degree in social work, psychology, or related fields with at least one year of supervised experience with seniors or disabled individuals; or 4+ years of relevant social services experience; or current IRIS Consultant in good standing
- Multilingual skills are a plus
- Strong commitment to participant autonomy and respectful service
- Comfort with smartphones, tablets, and computer systems
- Knowledge of social behavior, community resources, and diverse populations
- Valid driver’s license, reliable transportation, and a satisfactory driving record
