About the role

We are seeking an IRIS Consultant to support individuals in maintaining independence and managing long-term care services.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining regular contact and conducting visits
  • Assisting participants with IRIS eligibility and connecting with local agencies
  • Documenting all interactions accurately and timely
  • Potentially conducting annual functional screens
  • Collaborating with a supportive team while working independently

Qualifications and Skills

  • Bachelor's degree in social work, psychology, or related field, or equivalent experience
  • At least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
  • Knowledge of community resources and human behavior
  • Strong commitment to participant autonomy and respect for diverse populations
  • Proficiency with smartphones, tablets, and computer systems
  • Valid driver’s license, reliable transportation, and satisfactory driving record
