About the role
We are seeking an IRIS Consultant to support individuals in maintaining independence and managing long-term care services.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting visits
- Assisting participants with IRIS eligibility and connecting with local agencies
- Documenting all interactions accurately and timely
- Potentially conducting annual functional screens
- Collaborating with a supportive team while working independently
Qualifications and Skills
- Bachelor's degree in social work, psychology, or related field, or equivalent experience
- At least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
- Knowledge of community resources and human behavior
- Strong commitment to participant autonomy and respect for diverse populations
- Proficiency with smartphones, tablets, and computer systems
- Valid driver’s license, reliable transportation, and satisfactory driving record
