About the role

Join as an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining monthly contact and quarterly visits
  • Assisting participants with IRIS eligibility and connecting with local agencies
  • Documenting all contacts and visits promptly
  • Conducting annual Long-Term Care Functional Screens, if needed
  • Working independently while collaborating with a team

Qualifications

  • Bachelor’s degree in social work, psychology, human services, or related field with at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
  • Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
  • Strong commitment to participant autonomy and respectful service
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior, social interactions, and community resources
  • Willingness to serve diverse populations with empathy

Requirements

  • Valid driver’s license and reliable transportation
  • Satisfactory driving record and auto insurance compliance
