About the role
Join as an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining monthly contact and quarterly visits
- Assisting participants with IRIS eligibility and connecting with local agencies
- Documenting all contacts and visits promptly
- Conducting annual Long-Term Care Functional Screens, if needed
- Working independently while collaborating with a team
Qualifications
- Bachelor’s degree in social work, psychology, human services, or related field with at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Willingness to serve diverse populations with empathy
