About the role

IRIS Consultant supporting individuals in maintaining independence and self-direction through the IRIS program.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining regular contact and conducting visits
  • Assisting participants with IRIS eligibility and connecting with local agencies
  • Documenting all interactions promptly
  • Potentially conducting annual functional screens
  • Collaborating independently within a supportive team

Qualifications

  • Bachelor’s degree in social work, psychology, or related field with at least one year of experience working with seniors or individuals with disabilities, or
  • 4+ years of relevant social services experience, or
  • Current IRIS Consultant in good standing
  • Bilingual skills are a plus
  • Strong commitment to participant autonomy and respectful service
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior, social interaction, and community resources
  • Willingness to serve diverse populations with empathy

Requirements

  • Valid driver’s license and reliable transportation
  • Satisfactory driving record and auto insurance compliance
