About the role
IRIS Consultant supporting individuals in maintaining independence and self-direction through the IRIS program.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting visits
- Assisting participants with IRIS eligibility and connecting with local agencies
- Documenting all interactions promptly
- Potentially conducting annual functional screens
- Collaborating independently within a supportive team
Qualifications
- Bachelor’s degree in social work, psychology, or related field with at least one year of experience working with seniors or individuals with disabilities, or
- 4+ years of relevant social services experience, or
- Current IRIS Consultant in good standing
- Bilingual skills are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interaction, and community resources
- Willingness to serve diverse populations with empathy

- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
