About the role
Join as an IRIS Consultant and support individuals in living independently through the IRIS (Include, Respect, I Self-Direct) program.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining monthly contact and quarterly visits
- Helping participants understand and maintain IRIS eligibility
- Connecting with local agencies and ADRCs to expand services
- Documenting all contacts and visits promptly
- Conducting annual Long-Term Care Functional Screens (as needed)
- Working independently while collaborating with a supportive team
Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related fields, with at least one year of supervised experience working with seniors or people with disabilities; or 4+ years of direct social services experience; or current IRIS Consultant in good standing
Skills
- Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
- Strong commitment to participant autonomy and meaningful living
- Comfortable with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Respectful and empathetic toward diverse populations

- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
