Disability Services Consultant - Bachelor's Required

About the role

This position involves supporting individuals in maintaining independence and managing their care through the IRIS (Include, Respect, I Self-Direct) program. As an IRIS Consultant, you'll build trusting relationships, develop personalized IRIS Support and Services Plans, and assist participants in understanding their eligibility and available services.

Responsibilities

• Maintain regular contact with participants through monthly and quarterly visits
• Connect with local agencies to expand service options
• Document all interactions promptly within required systems
• Conduct annual Long-Term Care Functional Screens as needed
• Work independently while collaborating with a supportive team

Qualifications & Skills

• Bachelor's degree in social work, psychology, human services, or related field and at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience
• Current IRIS Consultant in good standing or DHS exemption preferred
• Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are a plus
• Strong commitment to participant autonomy and empathy
• Proficiency with smartphones, tablets, and computer systems
• Valid driver's license, reliable transportation, and satisfactory driving record
