About the role
Seeking an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Explain IRIS eligibility and connect with local agencies
- Document interactions accurately and timely
- Potentially conduct annual functional screens
- Collaborate with a supportive team while working independently
Qualifications and Skills
- Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Bilingual skills a plus
- Strong commitment to participant autonomy and empathy
- Proficiency with smartphones, tablets, and computers
- Knowledge of human behavior, social resources, and community systems
- Willingness to serve diverse populations
