About the role

Seeking an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.

Responsibilities

  • Build trusting relationships with participants
  • Develop IRIS Support and Services Plans
  • Maintain regular contact and conduct visits
  • Explain IRIS eligibility and connect with local agencies
  • Document interactions accurately and timely
  • Potentially conduct annual functional screens
  • Collaborate with a supportive team while working independently

Qualifications and Skills

  • Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
  • Bilingual skills a plus
  • Strong commitment to participant autonomy and empathy
  • Proficiency with smartphones, tablets, and computers
  • Knowledge of human behavior, social resources, and community systems
  • Willingness to serve diverse populations

Requirements

  • Valid driver’s license and reliable transportation
  • Satisfactory driving record and auto insurance compliance
