About the role

Supports individuals in maintaining independence and self-directed care through the IRIS program.

Responsibilities

  • Build trusting relationships with participants
  • Develop IRIS Support and Services Plans
  • Maintain regular contact and conduct visits
  • Assist with IRIS eligibility and connect with local agencies
  • Document contacts and visits promptly
  • Conduct annual functional screens as needed
  • Collaborate with a supportive team and work independently

Qualifications & Skills

  • Bachelor’s degree in social work, psychology, human services, or related field with experience, or 4+ years in social services with seniors/people with disabilities
  • Current IRIS Consultant in good standing or relevant experience
  • Multilingual skills a plus
  • Strong commitment to participant autonomy and empathy
  • Comfortable with digital devices and systems
  • Knowledge of community resources and social behaviors
  • Willing to serve diverse populations

Requirements

  • Valid driver’s license and reliable transportation
  • Good driving record and auto insurance compliance
