About the role
Supports individuals in maintaining independence and self-directed care through the IRIS program.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Assist with IRIS eligibility and connect with local agencies
- Document contacts and visits promptly
- Conduct annual functional screens as needed
- Collaborate with a supportive team and work independently
Qualifications & Skills
- Bachelor’s degree in social work, psychology, human services, or related field with experience, or 4+ years in social services with seniors/people with disabilities
- Current IRIS Consultant in good standing or relevant experience
- Multilingual skills a plus
- Strong commitment to participant autonomy and empathy
- Comfortable with digital devices and systems
- Knowledge of community resources and social behaviors
- Willing to serve diverse populations
Requirements
- Valid driver’s license and reliable transportation
- Good driving record and auto insurance compliance
