About the role
Join as an IRIS Consultant to support individuals in maintaining independence and self-directed care through the IRIS program. In this social services role, you'll build trusting relationships, develop IRIS Support and Services Plans, and assist participants in understanding and maintaining their eligibility.
Responsibilities
- Maintain regular contact with participants
- Connect with local agencies
- Document interactions promptly
- Conduct annual functional screens if needed
You will work independently while collaborating with a supportive team.
Qualifications
- A bachelor's degree in social work, psychology, or related fields
- At least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
- Bilingual skills are a plus
- Comfort with technology
- A valid driver’s license, reliable transportation, and meet auto insurance requirements
- A strong commitment to participant autonomy and empathy
