Disability Services Consultant - Bachelor's Required

About the role

Join as an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services. In this social services role, you'll build trusting relationships, develop IRIS Support and Services Plans, and conduct regular contacts and visits with participants. You will assist clients in understanding IRIS eligibility, connect them with local agencies, and document all interactions promptly. The position involves working independently within a supportive team, with travel across multiple counties.

Qualifications

• Bachelor's degree in social work or related fields with at least one year of experience working with seniors or people with disabilities, or 4+ years of direct social services experience
• Valid driver's license, reliable transportation, and meet auto insurance requirements
• Bilingual skills and familiarity with technology are pluses
