About the role
Provides compassionate care to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.
Responsibilities
- Assesses patient needs and develops personalized care plans
- Administers medications and treatments as prescribed
- Provides emotional support to patients and families
- Coordinates with healthcare providers and other caregivers
- Maintains accurate records and reports
Requirements
- Bachelor's degree in a healthcare-related field
- Current CPR certification
- Experience in hospice or home health care preferred
Qualifications
- Strong communication skills
- Ability to work independently and as part of a team
- Passion for providing compassionate care
Skills
- Excellent interpersonal skills
- Proficient in computer systems for documentation
- Knowledge of hospice and palliative care principles
