Advanced Practice Clinician - Per Diem

About the role

This position provides clinical leadership to enhance compliance with quality and care standards. It manages inter-professional teams delivering comprehensive advanced nursing care, including assessments, post-discharge visits, and urgent interventions. The role involves collaborating with patients, families, and healthcare providers to develop and adjust individualized care plans, ensuring adherence to regulatory standards and quality measures such as Patient-Centered Medical Home, HEDIS, and QARR. It includes participating in interdisciplinary team meetings, guiding team performance, and ensuring documentation and billing compliance. The nurse practitioner may perform procedures within collaborative agreements and provide peer reviews, precept students, and contribute to clinical projects.

For psychiatric roles, additional duties include psychosocial assessments, mental health treatment, crisis intervention, and collaboration with psychiatrists. The role requires maintaining licensure, certifications, and ongoing professional development.

• Ensure adherence to regulatory standards and quality measures (e.g., Patient-Centered Medical Home, HEDIS, QARR)
• Participate in interdisciplinary team meetings and guide team performance
• Ensure documentation and billing compliance
• Perform procedures within collaborative agreements
• Provide peer reviews, precept students, and contribute to clinical projects
• For psychiatric roles: conduct psychosocial assessments, provide mental health treatment, perform crisis intervention, and collaborate with psychiatrists
• Maintain licensure, certifications, and ongoing professional development

Qualifications

• Current RN and Nurse Practitioner licenses in New York State
• Specialty certification (adult, family, gerontology, or psychiatric-mental health)
• Master’s degree in Nursing from an accredited program
• Certification by ANCC or AANP; psychiatric NPs must hold PMHNP-BC certification
• Minimum of two years’ relevant clinical experience, including home care or managerial roles preferred
• Bilingual skills and a valid driver’s license may be required
