Young Adult Mental Health Case Manager (Lehigh Valley)
About the Role
Access Services is hiring a Young Adult Mental Health Case Manager (also known as a "TIP Facilitator") to join our Transition to Independence Process (TIP) program serving transition-age young adults (ages 16-26) across Lehigh Valley, with an office based in Bethlehem, PA. The TIP Program uses an evidence-based, strength-based, person-centered, and youth-driven model to support young adults in achieving their vision of a successful future.
TIP Facilitators empower young adults by helping them set and achieve goals, providing in-the-moment coaching, and facilitating problem-solving. This role involves delivering community-based services daily, building authentic relationships, and supporting young people in creating and following their personalized Futures Plan.

Responsibilities
- Deliver community-based services daily by driving to and meeting with young adults in their homes and community locations across Lehigh Valley.
- Engage youth using the TIP Model to build overall independence across five key domains: Education; Employment & Career; Living Situation; Community Life Functioning; Personal Effectiveness & Wellbeing.
- Build authentic, trusting relationships and support young people in creating and following their Futures Plan—a personalized roadmap of goals.
- Attend and advocate alongside young adults in multi-system meetings and collaborate with families, schools, employers, and community partners.
- Plan and participate in group events that promote connection, life skills, and fun.
- Complete documentation and meet billable hour requirements in alignment with program and regulatory standards.
- Participate in case management reviews, on-call rotation, and weekly team meetings.
- Represent the TIP Program through outreach and community engagement efforts.
Requirements
- Bachelor's degree in social work, psychology, criminal justice, sociology, social welfare, gerontology, anthropology, nursing, counseling, education, theology, or other related social sciences (required).
- Work or internship experience within mental health and community-based human services (preferred).
- Willingness and ability to travel and transport youth throughout the community (required).
- Valid US driver's license for at least two years (not including permit time) with no serious traffic convictions in the past three years (e.g., DUI, reckless driving, leaving the scene of an accident, or three or more traffic violations/at-fault accidents).
- Access to a reliable vehicle during work hours with valid registration, inspection, and auto insurance.
- Maintain regular and predictable attendance, including scheduled or unplanned overtime as necessary to meet program needs.
- Full-time, on-site presence required; no remote or hybrid work options.

- Ability to utilize engagement strategies to support youth with diverse mental health diagnoses, developmental needs, and varying abilities, including individuals with Level 1 and 2 Autism Spectrum Disorder.
- Ability to create and implement group event ideas for youth, ranging from simple to complex (recreational, skill-building, and community or peer connection).
Essential Working Conditions / Physical Demands
- Work in program sites and the community, routinely providing direct care and support.
- Stand, sit, or run to provide proper care and supervision.
- Able to move and stay with an individual to keep them safe, both in home and community settings, even when they struggle to meet expectations.
- Able to assist an individual who has fallen, regardless of weight or size.
- Walk, climb stairs, and provide direct care and support.
- Work in a standard office environment and utilize desktop computer equipment daily.
- Able to lift or move items associated with sedentary or light work (up to 20 lbs.).

Pay
- Starting pay: $20.50/hour, with opportunities for increases based on performance, education, and growth.
- Performance-based incentives available.
- On-call rotation stipend: $175/week.
Schedule
- Office hours: 9 AM–5 PM, with some evenings and weekends required. Flexible schedule as long as caseload needs and billable requirements are met.
- On-call rotation required once every 4–5 weeks.
