About the role
The Hampton–Newport News Community Services Board seeks a full-time Adult Mental Health Case Manager to support individuals in the Hampton and Newport News communities. This onsite role involves working alongside a collaborative team to deliver high-quality mental health services.
Position Summary
The Adult Mental Health Case Manager supports continuity of care for adults receiving behavioral health services across Hampton and Newport News. Key responsibilities include completing assessments, developing and updating treatment plans, providing advocacy and supportive counseling, coordinating community resources, documenting services, and responding to urgent situations with appropriate crisis intervention.
Required qualifications
- Bachelor’s degree in a Human Services field (e.g., Social Work, Psychology, Sociology, Counseling)
- One (1) year of case management experience
