Description Position Summary The Wellness Office Nurse Coordinator coordinates office-based nursing operations, records, and communication to support consistent and responsive wellness services. This role helps organize clinical workflows, follow-up tasks, and care coordination within the wellness office. Monitor workflow priorities and help ensure timely response to health-related needs. Prepare summaries, reports, and administrative materials related to wellness office operations. Assist with scheduling, care coordination, and process organization. Support quality improvement and operational consistency within the wellness office. Promote responsive, well-organized, and person-centered nursing coordination. Key Responsibilities Complete Annual Nursing Assessments, Self-Medication Assessments, Choking Risk Assessments, AIMS, and Fall AssessmentsComplete Focal AssessmentsComplete Comprehensive AssessmentsAdminister injectable medicationsPerform sterile proceduresMonitor Therap Health TrackingAssess and monitor wounds, perform dressing changesRemove staples and suturesMonitor medication administrationAdminister medications per prescriptions, standing orders, or verbal orders received from a physician as neededDevelop, implement, and follow through on plans of care for people in servicesSupervise and assess MAC workers as they perform assigned duties, ensuring adherence to protocolsBe available for periodic inspection and evaluation through physical presence, and electronic or telephonic communication for direction, consultation, and collaboration at all timesBe physically present in the facility and readily accessible to designate or prescribe a course of action or to give procedural guidance, direction, and periodic evaluationUpdate and maintain medication administration recordsCollaborate with other healthcare professionals to ensure that all orders are carried out as prescribedConduct group home auditsAttend in-service education programs, staff meetings, etc.Complete ER and Hospital GERs in accordance with documentation requirements and reporting procedures.Perform other duties as assigned by the supervisor to maintain compliance with ADMH, ABN, or Arc policy Requirements Qualifications Unencumbered RN license by the Alabama Board of Nursing.Completion of ABN-approved MAS Nurse training in ADMH Community Programs.CPR certification and CPI training.4-year bachelor's degree Skills & Competencies Review, update, and maintain medical recordsComplete and process medical documentationCollaborate with other healthcare professionals and pharmacies to ensure all prescribed orders are carried outCommunicate with team members, staff, and natural supports regarding the healthcare of the people servedReconcile person-served medications monthly and as neededComplete new admission paperworkPerform hospital follow-upConduct hospital dischargesReview Therap Health TrackingTrain MAC workers to perform assigned dutiesAdminister medications per prescriptions, standing orders, or verbal orders received from a physician as neededProvide MAC training Working Conditions Use of medical and office equipment including computers, phones, and medical tools.May require wearing protective gear such as gloves, masks, and lab coats.Physical activities include standing, walking, lifting, and performing medical procedures.Work is performed in clinical and office settings with occasional travel.
