As a Wellness Coordinator, you will play a crucial role in care coordination and administrative support for members with behavioral health and substance abuse needs.
Responsibilities
- Provide care coordination or administrative support to members with behavioral health and substance abuse needs.
- Work in an inbound and outbound member queue, document in multiple electronic documentation systems, coordinate information to clinical team partners, and provide behavioral health benefit education.
- Contact members to screen for and identify gaps in care, provide network referrals and appropriate community resources, and engage members in mental health and substance use treatment.
- Administer administrative support, potentially travel to provide direct patient care, and communicate directly with providers.
- Perform other duties as assigned.
Requirements
- High School Diploma/GED
- 2+ years of managed care or call center experience
- 1+ years of experience in Medical/Behavioral health setting
- 1+ years of computer proficiency experience to include Adobe and Microsoft Office
- 1+ years of experience handling sensitive issues with members and providers in a confidential manner
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Live in a location that can receive a UnitedHealth Group approved high-speed internet
Qualifications
- Bachelor's Degree in a social services
- Dual diagnosis experience with mental health, substance abuse, and special needs
- Knowledge of state waiver program
- Experience working in an environment that requires coordination of benefits and utilization of multiple groups and resources for patients
- Working knowledge of community-based resources and services
- Creative problem-solving skills with the ability to use community and network resources to meet members' needs
- Valid Missouri driver's license
Preferred Qualifications
- Casework experience at hospitals, social service agencies or community mental health centers
- Experience working in an environment that requires coordination of benefits and utilization of multiple groups and resources for patients
- Demonstrated creative problem-solving skills with the ability to use community and network resources to meet members' needs
- Demonstrated initiative in achieving individual, team, and organizational goals and objectives

Pay and Benefits
