The Wellness Case Manager at NIVA Health plays a crucial role in guiding patients through their wellness journey, from initial interactions with the clinic to ongoing programme engagement. Responsibilities
- Building strong, trust-based relationships with patients
- Sustaining patient engagement and wellness programme participation
- Guiding patients through provider-directed services and next steps
- Coordinating follow-ups, scheduling, and patient communications
- Creating a seamless, elevated patient experience
- Tracking personal KPIs related to engagement, follow-up activity, and conversion support
- Supporting new wellness initiatives and service rollouts within the clinic
- Collaborating closely with providers and clinic staff
