Responsibilities
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
- Provide clients with direct service, either in the office or in the field.
- Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination.
- Assist clients in developing formal and informal community support.
- Provide advocacy on behalf of clients.
- Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
- Attend clinical and administrative staff meetings, multi-disciplined team meetings as required.
- Review treatment plans and progress of cases with supervisor for continued support, advice, and approval.
- Perform other duties as necessary to accomplish objectives.
Requirements
- Combination of education and experience normally represented by a bachelor’s degree and one to two years related professional experience.
- Must have competent oral, written and interpersonal communication skills.
- Must be well organized, detail oriented and have the ability to maintain confidentiality.
- Able to work with diverse client population, community organizations as well as staff at all levels.
- Able to form good working and therapeutic relationships.

- Salary range: $44,349.15 - $48,231.52 annually
