Case Manager
Position Summary
The Case Manager serves as a vital member of the multidisciplinary treatment team, coordinating services and advocating for clients throughout their treatment journey. This individual will work closely with therapists, medical providers, family members, referral sources, and community partners to ensure continuity of care and successful treatment outcomes.
Essential Responsibilities
Conduct case management assessments.
Identify client needs and develop individualized service plans.

Coordinate referrals for medical, psychiatric, housing, vocational, educational, legal, and social support services.
Absorb
Assist clients with accessing community resources and benefits.
Facilitate discharge planning and aftercare coordination.
Communicate and collaborate with treatment team members regarding client progress and barriers to treatment.
Maintain ongoing contact with referral sources, family members, and external providers as clinically appropriate.
Monitor treatment compliance and assist clients in meeting program expectations.
Provide crisis intervention and support when necessary.
Maintain accurate, timely, and compliant documentation in accordance with regulatory and organizational standards.
Ensure compliance with HIPAA, confidentiality requirements, and all applicable behavioral health regulations.
Participate in treatment team meetings, case reviews, and clinical staffing.
Qualifications
Required:
Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field.
Minimum one (1) year of experience in case management in a behavioral health setting.
Strong understanding of community resources and care coordination practices.
Excellent organizational, communication, and interpersonal skills.
Knowledge of HIPAA and professional boundaries.
Ability to work effectively within a multidisciplinary treatment environment.
Preferred:
Certified Case Manager (CCM), Certified Addiction Professional (CAP), Certified Recovery Peer Specialist (CRPS), Certified Recovery Residence Administrator (CRRA) or other related credentials.
Experience working in PHP, IOP, residential, or outpatient behavioral healthcare settings.
Familiarity with KIPUBilingual (English/Spanish) a plus.
