About the role
The Wellness Case Manager at NIVA Health is responsible for guiding patients through their wellness journey, from initial interactions with the clinic to ongoing programme engagement. This role requires a strong focus on building trust, supporting patient engagement, and coordinating care.
Responsibilities
- Building strong, trust-based relationships with patients
- Sustaining patient engagement and wellness programme participation
- Guiding patients through provider-directed services and next steps
- Coordinating follow-ups, scheduling, and patient communications
- Creating a seamless, elevated patient experience
- Tracking personal KPIs related to engagement, follow-up activity, and conversion support
- Supporting new wellness initiatives and service rollouts within the clinic
- Collaborating closely with providers and clinic staff

- This is a full-time, on-site clinic role
- Rotating schedule may include occasional Saturdays (with a weekday off provided)
- This is a non-clinical role — all medical recommendations and treatment plans are directed by licensed providers
