About the role
Providing weekend home care services to patients in their residences.
Responsibilities
- Deliver direct patient care including personal hygiene, mobility assistance, and basic nursing tasks under supervision.
- Assist with transfers, repositioning, and ambulation to promote patient safety and comfort.
- Prepare meals tailored to patient preferences and dietary needs.
- Offer companionship and engage in activities suited to individual interests.
- Perform light housekeeping duties as needed.
- Document care activities accurately using EVV systems and maintain comprehensive records.
- Monitor and report changes in patient conditions, ensuring adherence to the Plan of Care.
- Demonstrate dependability through punctuality and respectful communication, maintaining patient confidentiality.
- Support patient independence within their capabilities and follow all safety protocols.
Qualifications
- Current certification with DFS.
- High school diploma or GED preferred.
- Effective communication skills, with the ability to read, write, and follow instructions.
- Physical ability to perform tasks including lifting (up to 50 lbs.), bending, and standing.
- Compassionate attitude toward patient care and respectful of diverse family environments.
