About the role
Certified Nursing Assistant (CNA) providing home care services in patients' residences.
Responsibilities
- Offering direct patient care, including personal hygiene, transfers, repositioning, and ambulation assistance.
- Administering basic nursing care and prescribed exercises under supervision.
- Preparing meals according to client preferences and special diets.
- Providing companionship and engaging in activities tailored to individual needs.
- Performing light housekeeping and maintaining accurate records of care provided.
- Monitoring and reporting changes in clients' physical or psychosocial conditions.
- Using EVV app or timesheets to document tasks, ensuring punctuality and dependability.
- Supporting client independence within their abilities and respecting their privacy and rights.
- Adhering to the Plan of Care and notifying supervisors of concerns such as abuse or unsafe conditions.
- Providing compassionate, direct care, documenting accurately, ensuring safety and dignity, and collaborating with healthcare team to deliver high-quality home care services.
Qualifications & Skills
- Maintain valid certification with DFS.
- High school diploma or GED preferred.
- Effective communication, reading, and following instructions skills.
- Ability to perform physical tasks, including lifting up to 50 lbs.
- Compassionate attitude toward patient care.
