About the role

This role involves providing direct patient care in clients' homes, including personal hygiene, transfers, repositioning, and ambulation assistance. The position requires travel within a designated area, with no guaranteed hours or assignment length. Physical demands and adaptability to varied patient needs are essential.

Responsibilities

• Administering basic nursing care and prescribed exercises under supervision.
• Preparing meals considering client preferences and dietary needs.
• Offering companionship and engaging clients in activities tailored to their interests.
• Performing light housekeeping duties and maintaining accurate records of care provided.
• Monitoring and reporting changes in clients' physical or psychosocial conditions.
• Completing tasks via EVV app or time sheets, demonstrating dependability and punctuality.
• Supporting client independence within their abilities and respecting family and client values.
• Upholding patient confidentiality, safety, and privacy, and reporting concerns, including abuse or neglect.

Qualifications

• Current certification with DFS required.
• High school diploma or GED preferred.
• Effective communication skills, including reading, writing, and following directions.
• Ability to perform physical tasks such as lifting up to 50 lbs, bending, and standing.
• Sympathetic attitude towards patient care.
