About the role

Provide in-home patient care, supporting clients with daily living activities in their residence.

Responsibilities

• Deliver personal hygiene care, including bathing, grooming, and mouth care.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises under supervision.
• Prepare meals considering client preferences and dietary restrictions.
• Offer companionship and engage clients in activities tailored to their interests.
• Perform light housekeeping duties and maintain accurate care records.
• Monitor and report changes in clients' physical or psychosocial conditions.
• Complete documentation via EVV app or timesheet; adhere to the plan of care.
• Ensure confidentiality, privacy, and uphold patient rights.
• Report any abuse, neglect, or safety concerns immediately.

Qualifications

• Maintain valid certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills, ability to read, write, and follow instructions.
• Physical ability to lift up to 50 lbs, bend, stand, and perform necessary tasks.
• Compassionate attitude toward patient care.

Additional details

The role involves travel within a designated area with no guaranteed hours or duration. Equipment used includes blood pressure cuffs and stethoscopes. The position emphasizes teamwork, dependability, and adherence to care plans.
