Responsibilities
- Deliver coordinated nursing care for a defined patient group according to established standards and the nursing process
- Collect relevant data, analyze assessments, and determine care needs; develop and implement a care plan; monitor and adjust to outcomes
- Cookice with a multidisciplinary team, involve patients and families in care decisions, and promote a safe, healing environment
- Supervise and direct nursing staff at multiple levels; delegate tasks appropriately and ensure continuity of care across shifts
- Administer medications and therapies, monitor responses, and address adverse events; maintain accurate documentation
- Participate in discharge planning and provide education to patients, families, and treatment teams; ensure effective communication and transition of care
- Utilize the EPIC EMR system for documentation and care coordination; maintain compliance with privacy and safety standards
Requirements
- Licenses/Certifications: Registered Nurse (RN) license in the state of practice (California) required; Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) from an approved vendor; Hospital Fire and Life Safety (HLFS); Neonatal Resuscitation Program (NRP) required
- Experience: At least two years of recent RN experience in the field
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Specialty/Setting: Ability to work effectively in a home healthcare - hospice environment; NICU experience is noted in the specialty
- EMR: Must have recent EPIC experience within the last 12 months
