Travel Registered Nurse Case Management Job
Overview
A Registered Nurse Case Manager position is available for a 13-week assignment starting September 21, 2026, in Chillicothe, Ohio. The weekly pay range is $2,376–$2,506, with 40 guaranteed hours per week, and a 5x8 Day shift (Mon-Fri, 7:30 a.m.–4:00 p.m.).
Responsibilities
- Assess discharge needs, social determinants of health, and discharge planning/referrals for Acute Medical and Surgical patients.
- Develop and implement safe, timely discharge plans and coordinate referrals to post-acute services.
- Collaborate with an interdisciplinary care team to support smooth patient transitions.
- Document care coordination activities in the patient record with accuracy and compliance.
- Manage a typical caseload around 20 patients per day, ensuring timely discharge planning.
- Communicate with patients, families, and care partners; provide education and resource linkage to support ongoing care.
Requirements
- 3+ years Acute Care Case Management experience.
- Must reside outside a 60-mile radius of the assignment location.
- Ability to work in a hospital-based setting.
- Strong communication and collaboration skills.
- Experience with discharge planning and addressing social determinants of health.
- Proficiency in clinical documentation and care coordination.
About the Role
This assignment features dayshift hours with no on-call, holidays, or weekends. It is hospital-based acute care case management for Acute Medical and Surgical patients.
