Travel Registered Nurse – Case Management assignment in Saint Helena, CA, starting 09/14/2026 for 13 weeks.
About the Role
Home health and hospice travel assignment supporting patient care in patients’ homes. EPIC experience required.
Pay
Weekly pay: $2,292–$2,357 with guaranteed 36 hours per week.
Schedule
5×8-hour days (day shift).
Responsibilities
- Plan, organize, and direct home care services to meet patients’ needs in their homes; manage a patient caseload effectively.
- Perform initial and ongoing nursing assessments; document patient status, response to treatment, and plan of care.
- Provide direct and/or indirect skilled nursing care within the plan of care; counsel and educate patient and family on nursing and related needs.
- Update nursing plans, medication profiles, and documentation promptly; ensure timely submission of all records per policy.
- Coordinate patient care with other disciplines; incorporate input from the care team and adjust the plan of care as goals are clarified.
- Communicate changes to goals or status with the patient and family, and serve as a resource to patients, families, and other care team members.
Requirements
- Licensure: Registered Nurse (RN) license in California.
- Certifications: CPR or Basic Life Support (BLS).
- Experience: At least two years of recent experience in the field.
- Education: Bachelor’s Degree in Nursing (BSN) preferred.
- Additional requirements: EPIC experience; valid Driver’s License and must be at least 21 years old; current automobile insurance and a reliable automobile; ability to work in a home health environment.
