Travel Registered Nurse Case Management Job

Responsibilities
- Conduct initial and ongoing comprehensive assessments of patients to identify nursing needs
- Develop and implement plans of care in collaboration with the patient and care team
- Provide direct and/or indirect skilled nursing care while coordinating with other disciplines
- Educate patients and family members on care plans, rights, and self-management strategies
- Document patient status, responses to treatment, and changes in plans promptly and accurately
- Monitor progress toward goals and adjust the plan of care as needed to optimize outcomes
Requirements
- Registered Nurse license in the state of practice (California) or as otherwise required by the state of assignment
- CPR or BLS certification from an approved vendor
- Valid driver’s license (at least 21 years old) and current automobile insurance
- Minimum of two years of recent RN experience in the field
- BSN preferred; acute care facility or home care experience preferred
- Ability to work effectively in a home health/care management-oriented setting as part of a multidisciplinary team
