Travel Registered Nurse Case Management Job

Overview

Registered Nurse – Case Management in Grand Junction, Colorado. Start date 07/27/2026 for a 6-week travel assignment. Weekly pay range: $1,874–$1,930 per week. Guaranteed hours: 0.0. Shift: Monday through Friday, 8:00 a.m. to 5:00 p.m. Work setting: home-based hospice care delivered wherever patients call home, with an interdisciplinary team approach to symptom management and goals of care.

Responsibilities

• Manage an assigned patient caseload in a home-based hospice program, coordinating care across settings
• Lead the interdisciplinary team in developing and updating individualized plans of care aligned with patient goals
• Conduct comprehensive assessments, monitor symptoms, and apply evidence-based symptom management techniques
• Educate patients and family caregivers about disease processes, goals of care, medications, safety, and caregiving tasks
• Document interventions, patient responses, and outcomes accurately; ensure smooth transitions between care settings
• Provide timely communication with the care team, and participate in weekend coverage and on-call responsibilities as needed

Requirements

• Active Colorado Registered Nurse (RN) license
• Graduation from an accredited nursing program (ASN or BSN)
• BLS for Healthcare Providers certification
• Experience in hospice, palliative care, or case management is preferred
• Ability to work effectively in a home-based care environment and travel to patient residences as required
• Strong assessment, communication, and critical thinking skills
