Responsibilities
- Plan, organize, and direct home care services to meet patient needs in the patient’s home
- Manage a patient caseload efficiently and coordinate care delivery across disciplines
- Provide patient care independently while collaborating with other core team members to achieve care goals
- Serve as a primary resource for patients, family members, and care team members
- Monitor patient progress, adjust the plan of care as needed, and facilitate timely access to services
- Serve as a primary resource for patients, family members, and care team members
- Support discharge planning and transition planning to optimize outcomes
Requirements
- Active Registered Nurse license in California
- Willingness to participate in a travel assignment and manage a home-based care caseload
- Prior experience in case management or home health care is preferred
- Strong communication and organizational skills; ability to work independently and with a multidisciplinary team
- Ability to work days and meet 40-hour expectations as needed
About the role
This 13-week travel assignment offers a weekly pay range of $2,633 to $2,695 with no guaranteed hours. Shift: 5x8 days (days). Work setting: travel assignment in a home-based care/case management capacity.
