Responsibilities
- Provide excellent patient care by supporting the development, implementation, revision, and reporting of the case management program
- Serve as a liaison among the patient, family, nurse, physicians, multidisciplinary teams, and the patient’s healthcare benefactor to optimize outcomes
- Act as a consultant to the healthcare team on specific patient items
- Coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge
- Ensure patients and families progress efficiently through the course of hospitalization and discharge planning
Requirements
- Active Registered Nurse license in California
- Experience in case management or care coordination (preferred)
- Strong communication and collaboration skills
- Ability to work in a hospital/acute care setting
- Knowledge of discharge planning and care transitions
