About the role

Hospital-based care coordination role for a Registered Nurse in Case Management. This position focuses on optimizing patient flow, developing case management plans, and ensuring seamless care transitions.

Responsibilities

  • Coordinate inpatient and outpatient processes within the admission and discharge workflow to optimize patient flow.
  • Assess patient needs and develop case management plans to support timely, appropriate discharge and resource utilization.
  • Collaborate with physicians, nurses, social workers, and external providers to coordinate seamless care transitions.
  • Facilitate access to services, referrals, and follow-up care; communicate barriers and progress to the care team.
  • Document care plans, interventions, and outcomes in the medical record; leverage health IT systems (Epic experience preferred).
  • Ensure compliance with regulatory standards and maintain a patient-centered approach throughout the care trajectory.

Requirements

  • Active Colorado RN license.
  • Graduation from an accredited RN program.
  • Minimum of 3 years RN Case Management experience.
  • Ability to work in a hospital-based setting and participate in rotating 12-hour shifts with weekend coverage.
  • Epic experience preferred; strong collaboration and communication skills.
  • Travel eligibility: permanent address must be at least 75 miles away from the assignment site.

Pay

Weekly pay range: $1,876-$1,974.

Schedule

  • Guaranteed hours: 36 hours per week.
  • Shift: 3x12-hour rotating days; full-time daytime schedule with weekend rotation; no on-call.
